Basic Life Insurance

CHISLHB provides life insurance and accidental death and dismemberment coverage to all regular full-time employees for an amount equal to your basic annual earnings to a maximum of $50,000.

Voluntary Life Insurance

Regular full-time employees may elect to purchase additional life insurance for themselves and their family. Premiums are paid through post-tax deductions. Benefits available are shown here:

Voluntary Short Term Disability

Short Term Disability is intended to protect your income for a short duration in case you become ill or injured. All full-time active employees working 30 or more hours per week in an eligible class are eligible to apply for coverage.

Coverage  $50 increments up to $1,000 per week,
not to exceed 60% of pre-disability earnings. 
Maximum Benefit Duration  22 Weeks
 Benefits Begin  31st Day Accident
 31st Day Illness
 Pre-existing Condition  You may not be eligible for benefits if you have received treatment for a condition within the past 12 months until you have been covered under this plan for 12 months.
